NOTE:  In order to renew a certificate, you must have an approved IPDP on file with the LPDC PRIOR TO TAKING ANY COURSEWORK OR EARNING ANY CEU HOURS.

To file an IPDP with the LPDC: 

 Print and complete this form and turn it in to your building 
representative to file the plan with the LPDC.
